2024 Healthiest Companies Award Application

First Coast Worksite Wellness Council wants to recognize your efforts to support your employees' health and wellbeing.

All applicants must complete the Healthiest Companies Award application in its entirety. No recertification opportunities will be offered.

We are now accepting applications for the 2024 Healthiest Companies Awards!

How to Apply:

For Non-members: You must be a member to be eligible for consideration. Click here to fill out the application form on our membership page. After membership is approved and payment is received, you can follow the Member Instructions below to apply for the 2024 Healthiest companies Award. 

For Members: Once applications are open, please follow these steps to apply:

• Click the 'Log In' button at the top right section of this page. Enter the email address your membership is registered under and your password to log in. 
• • If you are unsure of the registered email address for your organization, email awards@firstcoastwwc.org to request this information.
  • If you do not have a password, click 'Forgot Password' to create one.
• To pay any outstanding membership fees, click your name at the top right section of the page to access your profile and view invoices (Awards cannot be distributed unless all membership fees are paid in full).
  
• After you log in, you will have access to the "HC Application" page under the "Members Only" tab. The applications will be posted to this page.
• Select the 2024 Healthiest Companies Application. 
• Applications are to be completed and submitted online. Please plan to complete your application in one session! Responses will not be saved. However, a PDF version of the application will be available after you log in to plan your responses before you begin the application.

What Award Level Should I Apply For?

Below you'll find a description of the type of wellness program associated with each award level. 

Bronze Level - Bronze award winners have started to make small changes to their workplace to encourage employees to improve their health and well-being. Examples of these changes may include sending communications promoting healthy lifestyle behaviors, encouraging employees to complete an annual wellness visit with their physician and promoting a culture of health through fun on-site activities and community events.

Silver Level - Silver award winners have established more of a foundation for their wellness program. In addition to promoting a healthy lifestyle through communications and on-site activities, they've also established wellness champions and allocated funds toward the implementation of their program. They also offer programs that address other dimensions of wellness, not just physical well-being.

Gold Level - Gold award winners have dedicated additional resources to ensure that their programs are being properly measured each year. They conduct an annual employee needs and interest survey, have a wellness operating plan and strategically select programs that will address their most prevalent health risks and conditions. 

Platinum Level - Platinum award winners have best in class wellness programs and have truly created a culture of health and well-being throughout their organization. They have full leadership support and invest in necessary resources to strategically design, implement and measure programs that show demonstrated improvement in the health and well-being of their employees. Platinum-level award winners bolster total well-being by demonstrating robust programming, supporting of multiple pillars of well-being (ie. Physical, mental/emotion, social, career, financial, etc.).

The sections of the Healthiest Companies award application are based on the key components of a well-rounded wellness program. This includes:

• Wellness Collaboration and Program Design
• Wellness Programs and Initiatives to Support Well-being
• Evaluating and Measuring Opportunities and Successes
